Parental Consent Required for SAT Questionnaire

The following Priority Notice was sent on February 26, 2016 

The College Board provides an optional Student Data Questionnaire for third year high school students taking the SAT in Maine. In accordance with the Family Educational Rights and Privacy Act (FERPA), the Maine Department of Education is requiring all schools participating in the April 12, 2016 SAT School Day Administration to have Informed Parental Consent prior to issuing the Student Data Questionnaire to any student.

While all third year high school students are required to participate in the state assessment, they are not required to participate in the Student Data Questionnaire.

Please distribute the Informed Parental Consent form to all participating test-takers. All signed forms must remain on file in your school or district; do not return these to the Maine Department of Education, the College Board, or Educational Testing Service.

Upon a student’s return of a signed form, please provide the Student Data Questionnaire and submit according to instructions. Alternatively, students may choose to create or update their student profile and opt-in to Student Search Service online at https://student.collegeboard.org/.
