The Maine DOE Office of Special Services has revised the forms listed below. Revised forms are to be used effective August 1, 2015.
- Written Notice – corrected phone number
- Individualized Education Program – changed order of functional, academic and academic sections; added regulatory language references
- Learning Disability Evaluation Report – revisions include guidance on using form with English language learners and the need to use more than a single measure
- Seven Day Waiver Form – an optional form that SAUs may use as an attachment to the Written Notice in which parents provide a signature agreeing to waive the 7 day notice provision
The Office of Special Services will provide the revised forms to vendors. We will also offer a webinar in September to further explain the form changes.
