Office of Special Services notice of changes in special education forms

The Maine DOE Office of Special Services has revised the forms listed below. Revised forms are to be used effective August 1, 2015.

  • Written Notice – corrected phone number
  • Individualized Education Program – changed order of functional, academic and academic sections; added regulatory language references
  • Learning Disability Evaluation Report – revisions include guidance on using form with English language learners and the need to use more than a single measure
  • Seven Day Waiver Form – an optional form that SAUs may use as an attachment to the Written Notice in which parents provide a signature agreeing to waive the 7 day notice provision

The Office of Special Services will provide the revised forms to vendors. We will also offer a webinar in September to further explain the form changes.
